I think it has to be done by "feel". In some areas the predominant custom will be to use "natural" names (Sun, Moon, Light, Dark etc), while others will have been named after a person, in which case the name will be fictional (Ithrorn, Kor, Marakith, Ithril).
Sticking to one or the other doesn't work, nor does a percentage approach. You have to apply it in areas, because names come from cultures.
For instance - and this is a very minor criticism - the names in Sun's End are difficult to remember, precisely because there are so many of them on the same theme. You must avoid this at all costs on a big map.

Well, here are domain names I have so far employed, covering about a quarter of the map:
Kroanon, Targasso, Tilmun, Nirn, the Damned, Vorhartz, Hartz, the Lake, Faltara, Pelvir, Enwind, Northwind.

Yes: try to capitalize on fragments. Hartz, Volhartz, Midhartz. Kanor, Olkanor, Mithkanor, Kanorim, Kanor-al-kanor. Stuff like that
